Care Management

1

Do you create a personalized care plan for each client? What does this include?

Yes. Every client receives a personalized care plan that includes their goals of care, a full client profile, care tasks, emergency information, and schedule. The care plan helps ensure consistent, individualized support tailored to each client's needs and preferences.

2

How often do you evaluate and update a client’s written plan of care?

We review and update care plans every 90 days, or sooner if there is a change in the client's needs, health, preferences, or schedule. Care plans are flexible and designed to evolve over time to ensure each client receives the right level of support.

4

What is your policy for communicating with a client and their family members?

We believe open communication is essential to providing exceptional care. We check in with clients and their designated family members or representatives after 1 day, 1 week, and 1 month of service, and then every 90 days thereafter. We are also available between scheduled check-ins and communicate regularly by phone, text, or email to ensure everyone stays informed and supported.

Can care start quickly if needed?

In many cases, yes. We work to begin services as soon as a caregiver match is available.

5

How do you handle emergencies?

We follow a standard emergency protocol. In the event of an emergency, caregivers will call 911 first, then notify the client's emergency contacts and the agency owners. If there is any uncertainty about the situation, we encourage caregivers to contact the client's emergency contacts for guidance and support.

Who answers after-hours phone calls?

After-hours calls and texts are answered directly by our owners, not an answering service.
